The Two Routes
Route A: Fly JB → Hat Yai, then Pak Bara Ferry (Fastest, Year-Round)
Fly from Senai Airport (JHB) to Hat Yai (HDY) — approximately 1 hour, from ~$18 USD on AirAsia. From Hat Yai, take a minivan to Pak Bara pier (~1.5–2 hours, ~$6 USD) and a speedboat to Koh Lipe (~1.5 hours, ~$15 USD). Total door-to-beach time: approximately 5–6 hours.

Route B: Overnight Bus JB → Hat Yai, then Pak Bara Ferry (Budget)
Take the overnight bus from Larkin Terminal to Hat Yai (~12 hours, ~$15 USD), then minivan to Pak Bara and speedboat to Koh Lipe. Total journey: 15–18 hours. Cheapest option at around $35 USD all in, but a long day — best done with an overnight stop in Hat Yai to break the journey.

Route C: Via Langkawi Ferry (High Season: Oct–May Only)
In high season, you can add a Langkawi stop: fly or bus from JB to Penang or KL, ferry to Langkawi, then ferry to Koh Lipe. This turns the journey into a 2–3 day island-hopping trip rather than a transit. Only realistic if you have time — not an efficient route.
